Home Care - Scope of Services
- 01Our residential program is focused on helping individuals achieve the greatest level of independence possible. We provide services to individuals with neurological, developmental, physical, and mental health conditions. Our program offers a range living options from beautiful home-like settings to supported apartments where residents work on developing the skills for greater independence with support of our caring staff. Every person has a unique set of goals and abilities. Our services are designed to be highly individualized but always focused on helping each person reach their greatest social, emotional, and functional potential.
- 02We offer 24-hour customer care 365 days a year at group home living and by service plan for residential apartments.

- 04As a locally operated 501(c)(3) non-profit organization, we are committed to the individuals, families, and communities we serve. We work directly with each person served, their family, and a team of healthcare professionals to ensure needs are met through a highly individualized service plan. Stakeholders may include the individual, family member(s), guardians, referral facility discharge planners, medical providers, case managers, and adjusters. WSC’s residential services include two distinct program types: • The Howell Residential Home and Supported Apartments serve individuals with traumatic brain injury (TBI) and emphasize cognitive, behavioral, and physical rehabilitation in a community-integrated setting. • The Genoa Township Group Home provides a supportive environment for individuals with physical disabilities, developmental disabilities, and/or mental illness, focusing on daily living assistance, wellness, and stability. Each person’s service plan is developed collaboratively and is reviewed at least annually or whenever a significant change in health or functional status occurs. The plan outlines supports in areas such as personal care, medication management, mobility, transportation, socialization, and community involvement. Our direct support professionals assist with activities of daily living (eating, toileting, bathing, grooming, dressing, transferring), reminders for appointments, support for social-emotional needs, and other individualized goals. For individuals with TBI residing in the Howell group home or apartments, WSC incorporates the Mayo-Portland Adaptability Inventory (MPAI) to monitor functional progress and support goal development. The MPAI is administered at regular intervals to track changes in adaptability, cognitive and physical functioning, and social integration, and is used to inform service planning and discharge readiness.
- 05WSC staff backgrounds include master and bachelors’ educational degrees; experience in the areas of vocational rehabilitation, vocational rehabilitation counseling, teaching, social work, human resources, HR development of youth and adult services, geriatric services, certified brain injury specialist, business administration, staffing recruitment, psychology, service industry, and management. In addition, staff participates in annual job specific training in Recipient Rights, Blood Borne Pathogens, Person Centered Planning, Grievance & Appeals, Deficit Reduction, Limited English Proficiency, CPR/First Aid, and Medication Administration Review, which includes distribution and storage of medication only allowable following the written orders of a licensed physician for the purpose of maintaining health and treatment.
- 06Howell Group Home & Apartments: Adults with Traumatic Brain Injury (TBI). Genoa Group Home: Adults with physical disabilities, developmental disabilities, and/or mental health diagnoses. All individuals must be medically stable and not in need of skilled nursing-level care.
- 07Authorization for service form is completed by referring agent, individual or family member to the WSC Employment Services Manager. Referrals can be made to (810)534-6116 Anita or (810)534-6163 Amy.
- 08Self-referrals are accepted as well as referrals from family members, physicians or health and human service providers such as, service coordinators, therapists and/or hospital social workers, Michigan Rehabilitation Services, automotive and medical insurance carriers, and Community Mental Health.
- 09Individuals are referred for admission to services through Community Mental Health, Michigan Rehabilitation Services, Medical and Vocational Case Managers, self-referrals, families, by other service providers, such as service coordinators, therapists and/or hospital social workers.
- 10An authorization or written service referral form needs to be completed which specifies the expected outcomes, amount, frequency and duration of services. Depending on the service authorized, a participant may also need a physician’s script.
- 11The involvement of family members, legal guardians, and natural supports—as identified by the person served—is central to service planning and success. WSC ensures these partners are: • Invited to participate in the development and review of service plans • Included in discharge and transition planning • Offered education, counseling, and advocacy support • Provided remote participation options (e.g., phone, video call) if in-person meetings are not feasible WSC values family input and strives to maintain open communication throughout the person's time in the program.
- 12We accept most insurance policies (e.g. medical, auto, worker’s comp, etc.) Michigan Rehabilitation Services, Community Mental Health and private pay. We also receive funds through the Work Skills Foundation.(https://www.wskillsfoundation.org/)
- 13Fees for service are based upon ability to pay or coverage such as private pay, insurance carriers, Community Mental Health, Michigan Rehabilitation Services or Department of Human Health Services. Service rates vary on the service needed and can be obtained upon request.

- 16WSC Home Care, formerly Action Home Health Care (AHHC), specializes in personal care services to customers who are faced with illness, recovery or diminished ability to care for themselves. All services are carried out directly by WSC Home Care staff using a person-centered approach to help ensure positive outcomes for patients and customers. Care is provided to individuals in their homes and community-based settings.

- 20We offer services to take care of life’s daily needs and tasks as needed by the individual. Our dedicated team of caregivers provide a wide range of personal care services as outlined below striving to ensure and / or promote independence. Services offered: • Supervision • Safety Monitoring • ADL assistance • Community Access and Integration • Nutrition/Meal Preparation • Personal Care • Medication Management • Home Keeping • Transportation • Team Meetings Additional services available by request. As a locally operated 501 C3 non-profit organization, we are committed to the individuals, families, and communities we serve. We work directly with you, your family and often a team of other health care professionals to make sure your needs are met. Each client/participant has a personalized service plan developed with input from all stakeholders and implemented by our team of professionals. Stakeholders may include the individual, family member(s), guardians, referral facility discharge planners, treating medical team, case managers and adjusters. Progress is continually measured, and plans are often updated to ensure that our customers’ needs are met. Hours of service are based on the person’s individual plan of service and are scheduled in a 4-hour block of time or more.

- 22WSC Home Care provides services for individuals requiring assistance related to home health needs and barriers to independence in the community.
- 23WSC offers home health care services in an individual’s current community based residence. The setting of the program depends on the identified needs and goals of the person served.
- 24Staff are available 24 hours a day, 7 days a week to provide support and supervision in all settings. The level of staff support and frequency of services is determined by the treatment needs of the consumer as outlined in the plan of care.

- 28Depending on the service authorized a participant may be required to provide a social security card, physician’s script, documentation of medical necessity, authorization for services, state ID card, or birth certificate.
- 29We accept most insurance policies (e.g. medical, auto, worker’s comp, etc.) Michigan Rehabilitation Services, Community Mental Health, and private pay. A fee scale is available upon request to all referral and funding agencies. A detailed description of services to be provided and fees is sent to all funders in the form of a Billing and Verification Letter prior to the start of services. A copy of this letter is to be kept in the Client File for reference. Communication of a change in fee structure is communicated to all funding sources through a Rate Change Letter. A copy of this letter is kept in the Client File for reference.
- 30Fees for service are covered by either worker’s compensation, private pay, insurance carriers, Community Mental Health, Michigan Rehabilitation Services or Department of Human Health Services. Service rates vary on the service needed and can be obtained upon request.
