Customized Services

As a locally operated 501(c3) non-profit organization, we are committed to the individuals, families and communities we serve. We work directly with you, your family and often a team of other health care professionals to make sure your needs are met.  Each client/participant has a personalized service plan developed with input from all stakeholders and implemented by our team of professionals. Stakeholders may include the individual, family member(s), guardians, referral facility discharge planners, treating medical team, case managers and adjusters. Progress is continually measured and plans are often updated to ensure that our customers’ needs are met.  Our caring, trained, bonded and insured staff is dedicated in providing the highest quality of care to individuals on an hourly or visit basis. Hours of service are based on the person’s individual plan of service.

Admissions & Referrals

WSC Home Care, formerly Action Home Health Care Services, is specifically designed for adults who need non-medical assistance with activities of daily and household support services. We understand how overwhelming the choice is to accept assistance and select a qualified home care provider.  We provide a face-to-face consultation with you and your family members to discuss your unique needs and help you determine care options. Our full service/concierge in-home care services are planned for customers to decide on the supports s/he needs to maximize their life journey.​

Each referral is closely monitored for potential admission. At the completion of the process and verification of funding, the WSC Home Care Supervisor will inform the individual of his or her acceptance status.

Each year, our employees participate in a variety of community activities and support a number of worthwhile organizations/causes.  Profits received through our services are directed back to our organization to support and benefit our participants.
